Palliative Care Manager
The palliative care manager is responsible for managing the palliative care service to include supervision of applicable staff, palliative care activities and patient flow. This person will track and report on identified quality metrics, leading performance improvement and various administrative functions. The palliative care manager works in collaboration with the medical director or designee to facilitate the interdisciplinary needs of patients and their families. This position will also collaborate with the primary care manager and director of clinic operations to ensure seamless transitions of patients.
