Jessica Reier is the Director of House Calls, where she leads the primary and palliative care programs. Her focus is on ensuring that the highest standards of care are consistently met and exceeded.

Dr. Mindy Shah is a Physician trained in internal medicine, geriatrics, hospice and palliative medicine.

Dr. Lori Kuehne has been a Family Physician for 30 years. She is board certified in family medicine, hospice and palliative care.

Melissa Fofana is a Nurse Practitioner with a passion for providing in-home assistance and personalized care for her House Calls patients and their families.

Leah Neugebauer has been a Nurse Practitioner for over fourteen years. Leah is guided by a strong sense of purpose and passion for caring for others.

Ashley Murrey has been a dedicated Nurse Practitioner for over fourteen years. She has experience working in the ICU, Oncology and Family Practice.

Caleb Christiansen is an RN Intake Coordinator, helping assist with gathering patient information and scheduling provider appointments.

Laurie Asmus is an Intake Coordinator with over seven years of experience working in healthcare. She enjoys visiting with patients and working closely.

Sarah Domier is a Nurse Practitioner with experience working in various settings, including the Cardiac/Progressive Care Unit, ICU and Endoscopy Department. She’s excited to support patients in living and enjoying their lives to the fullest.

Alicia Wolf is a visiting provider with extensive experience providing comprehensive geriatric care to the community. She is dedicated to advancing geriatric nursing education and serving leadership roles, including President of the Ohio Chapter of the Gerontological Advanced Practice Nurses Association.
